I (participant) acknowledge that the use of a motion platform, although considered safe, does involve a moving mechanical device. As with all such devices, there is a potential risk of serious injury and/or death and/or property damage.
The Participant is participating in this activity of their own volition and understands and assumes the risks associated with it. THE PARTICIPANT RELEASES, WAIVES, DISCHARGES AND COVENANTS NOT TO SUE THE OPERATOR AND DOF REALITY CORP., INCLUDING ITS PARRENT, SUBSIDIARIES, AFFILIATES, OFFICERS, DIRECTORS, EMPLOYEES, AGENTS, AND VOLUNTEERS (“RELEASEES”) FROM ALL LIABILITY TO THE PARTICIPANT, their personal representatives, assigns, heirs, and next of kin FOR ANY AND ALL LOSS OR DAMAGE, AND ANY CLAIM OR DEMANDS THEREFOR ON ACCOUNT OF INJURY TO THE PERSON OR PROPERTY OR RESULTING IN DEATH OF THE UNDERSIGNED ARISING OUT OF OR RELATED TO THE ACTIVITIES OF MOTION SIMULATION.

I also acknowledge that I may be photographed, filmed or otherwise recorded while in the event space. I agree to grant DOF Reality Corp. unlimited and unrestricted rights to use these recordings (including my likeness, image, voice, and appearance) for any promotional purposes, and without additional consideration or compensation. I waive any right to inspect or approve any such materials.
